City Hospice Trust Limited
January 2026
City Hospice is Cardiff’s local hospice, a registered charity and since 1987, have been providing free specialist palliative and end of life care services for people living with terminal illness, including cancer, motor neurone disease, heart failure, chronic respiratory illness, dementia and frailty. We are well-known for our outstanding care and for delivering projects that extend the reach of our work to make an even bigger difference for local people.
Our care and support is delivered by an expert team of medical consultants, specialist nurses, paramedics, counsellors, occupational therapists, complementary therapists and welfare benefits advisors. We work in people’s homes, in care homes, in outreach settings across Cardiff, and in our outpatient centre, Ty Hosbis, in Whitchurch.

The diagnosis of a terminal illness changes everything, leaving people feeling frightened and taking away hopes and dreams for the future. We focus on building trust and rapport and understanding what matter most so they feel confident to turn us when they need our help and support. We provide symptom control, emotional support, and practical help so people can live life fully, and when the time comes, die peacefully in the place they call home. People describe our support is a lifeline.
The need for our services has never been greater. In 2025 we will provide direct care services for over 1700 patients. At any one time, our team is looking after at least 550 people. Each month more than 110 new patient referrals are received and over 80 of the people we are caring for will die.

We will use a 3-year Tier 2 grant to expand our Day Hospice and Wellbeing Services so more people can get the support they need to live life as fully as possible. Specifically we will use the funding for key project roles to:
1. Improve the quality of life for people with terminal illness in Cardiff and their carers
2. Be accessible across our diverse community
3. Increase patient and carer control, choice and independence to boost confidence and coping
4. Promote mental, physical and emotional wellbeing
5. Reduce the loneliness and isolation experienced by patients and living with terminal illness and their carers.

Our project will have the hallmarks of City Hospice’s locally unique care – heartfelt, personalised, easy to access and lifelong, and will be safe, effective, responsive, caring and well-led.
We will gather and share feedback in real time throughout our project so we can make changes to maximise impact and value.
